Hot Chocolate in a Jar

In 2003 I had a recipe for making a layered hot chocolate mix in a jar.  I have lent the book it was in out and cannot remember who has it!  Does anyone have a source for this?

What is Snowmans soup? I might be interested in that idea. Please excuse the differece in the print, I pushed a button and that is what happened. I'm still learning this. Thanks, Annette
